Job Summary
Partner with care team members to develop and implement patient-centered clinical plans for adults and seniors with high medical and social complexity. Deliver wrap-around services through home visits, performing key clinical tasks including chronic condition support, wound care, phlebotomy, and medication review. Engage hard-to-reach patients directly at home while ensuring continuity of care through coverage for team members when needed. Maintain independent home visits with a minimum of 5 years of driving experience and valid vehicle insurance. Base salary ranges from $28.00 to $32.00 per hour with an annual bonus, plus full healthcare coverage, 401K match, and 15 days of PTO.
Required Qualifications
- Licensed Practical Nurse (LPN) licensure in the state of employment
- Ability to conduct independent home visits
- Experience providing care for the whole person, including caring for adults and seniors with high medical and social complexity
- Current CPR and BLS certifications
- Intermediate to advanced computer and software skills, including Patient Health Exchange Platforms, Electronic Health Records, and Outlook
- Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation
- Proof of liability and property damage insurance on vehicle used
- Licensed for a minimum of 5 years
- No vehicle-related suspensions, DUI, reckless driving, leaving the scene of an accident, vehicular manslaughter, felony convictions, or 3 or more moving violations within the last 5 years
- No more than 2 moving violations or 1 at-fault accident in the last 3 years
- Fully vaccinated and to provide records for validation
