Job Summary
Provides in-home nursing care for pediatric patients within the hospital-at-home program; uses the nursing process to assess needs, plans and implements individualized home interventions, and evaluates care effectiveness. Collaborates with the health care team, coordinates discharge planning, ensures patient safety, applies evidence-based practice, and demonstrates leadership while delivering care in the home setting. Requires RN licensure, accredited nursing education, and BLS; valid driver's license and ability to travel within the designated geographic area. Pediatric Advanced Life Support (PALS) preferred. 24 hours per week, 8a-8p shifts, 2 days per week with every 3rd to 4th weekend; includes in-home assessments, family involvement, and coordination with virtual care team.
Required Qualifications
- Registered Nurse license (state of practice)
- Completion of an accredited nursing program
- Basic Life Support (BLS) certification for Healthcare Providers
