Field Hospital Enrollment Specialist (Bilingual Spanish)

Review daily patient census reports and partner with hospital teams to identify and prioritize eligible Medi-Cal and Medicare patients. Conduct in-person bedside outreach in emergency departments, observation units, triage areas, and inpatient units to verify eligibility, educate families about the Enhanced Care Management program, and enroll consenting patients before discharge. Document all outreach attempts, attestations, and enrollment activity in the electronic medical record while coordinating warm handoffs and pending follow-up needs with hospital partners. Participate in shift huddles and department check-ins as needed, building strong relationships with registration, social work, nursing, and unit leadership. Escalate census delays, access issues, and workflow barriers to the appropriate leader. This on-site role is based in Fresno, Stanislaus, or Los Angeles County hospitals, requiring bilingual fluency in English and Spanish, a valid driver's license, and reliable transportation.
