Job Summary
Supervise Community Health Workers by allocating work, resolving problems, and leading team huddles while carrying a 50% to 75% caseload. Provide daily peer support and resource navigation to members in clinics, facilities, or homes, identifying social determinants of health and collaborating with care managers to implement individualized care plans. Document program workflows, policies, and outcomes through field data collection, participant surveys, and chart reviews. Assist CHWs with maintaining certifications and continuing education, facilitate care coordination for hard-to-reach populations, and help members access programs and appointments. Lead in field data collection and submit reports to the Program Manager. This mobile role requires regular travel to various locations, including homes and offices, with a starting salary of $55,000 to $60,000.
Required Qualifications
- High School Diploma or General Education Diploma (GED)
- Minimum of two years leading a team of 2 to 4 employees
- Ability to attain Community Health Worker certification within 1 year of hire
- Must possess/maintain a valid driver's license
- Current automobile insurance
- Driving record that meets the guidelines and requirement of the organization
- Must be able to prioritize and meet deadlines
- Detail oriented
- Exercise good judgment
- Above average organizational skills
- Good listener
- Team player
- Must be able to effectively lead and train employees
- Ability to organize, prioritize and scheduling work assignments
- Minimum of two years of experience in either volunteer or paid position working in community setting with at risk populations providing coordination of services
- Must be available for regular travel to different work locations
- Must be able to stand and/or sit for long periods of time
- Must be available for weather conditions typical of the location
Desired Qualifications

- Bilingual (Spanish)
- Previous experience servicing diverse populations in rural areas
