Job Summary
Facilitate face-to-face care coordination for injured workers and medical providers across North West Ohio, acting as a liaison to advocate for progressive treatment and timely return-to-work. Identify appropriate providers and facilities throughout the continuum of services while ensuring resources are used cost-effectively. Maintain a home-based office while managing complex medical conditions, treatment planning, and recovery from illness or injury. Requires RN licensure preferred, Bachelor's degree, and valid driver's license. Full benefits including medical, dental, vision, and 401K begin on day one.
Required Qualifications
- RN licensure preferred
- Bachelor's degree in health or human services field required
- Valid driver's license required
- High speed internet required
- Employment may be contingent upon successful completion of a motor vehicle record (MVR) check
- Clear and conceptual thinking ability
- Excellent judgment
- Troubleshooting, problem solving, analysis, and discretion
- Ability to handle work-related stress
- Ability to handle multiple priorities simultaneously
