Job Summary
Support the delivery of the highest quality care in the community by providing personal care with dignity, completing assessments and reviews of individual care services, and carrying out regular needs and risk assessments. Provide practical supervision, instruction, and support to Care Assistants while working with Care Coordinators to plan effective rotas. Shadow new care workers to ensure their competence and observe practice during routine supervision or performance reviews. Complete essential administrative tasks linked to care delivery and service quality. Requires a full UK driving licence and access to your own vehicle for local travel. Full training and paid induction provided with flexibility in work patterns.
Required Qualifications
- Full UK driving licence
- Access to your own vehicle
- Experience carrying out initial risk and needs assessments for individual service users, either in their own homes or in other settings such as hospitals or residential/nursing homes
- Experience working in partnership with service users, their families, carers, advocates, healthcare professionals and other stakeholders to ensure seamless, holistic care
- Experience supporting high-quality care delivery and contributing to safe, person-centred care planning
- Experience communicating clearly, professionally, and with compassion when coordinating care and reviewing needs
