Job Summary
Join Molina as a Field Care Manager (RN) to provide support in care management and care coordination for members, focusing on assessing needs, developing care plans, and facilitating service integration. The role includes performing comprehensive member assessments, developing personalized care plans in collaboration with health professionals, and monitoring the effectiveness of care strategies. Candidates should possess solid experience with Microsoft Office, strong communication skills, and at least two years of healthcare experience, including care management.
Required Qualifications
- At least 2 years experience in health care
- At least 1 year experience in care management, managed care, or in a medical or behavioral health setting
- At least 1 year experience working with persons with disabilities, chronic conditions, and long-term services and supports (LTSS)
- Registered Nurse (RN) with active and unrestricted license
- Valid and unrestricted driver's license
- Reliable transportation and adequate auto insurance for job related travel
- Ability to operate proactively and detail-oriented work
- Demonstrated knowledge of community resources
- Ability to work independently with minimal supervision
- Excellent time-management and prioritization skills
- Strong verbal and written communication skills
- Microsoft Office suite/applicable software program(s) proficiency
Desired Qualifications
- Certified Case Manager (CCM)
- Experience working with populations that receive waiver services
Additional Requirements
- At least one year of experience working directly with individuals with substance use disorders may be required in some states
