Field Care Manager, LTSS (LVN, LBSW or LMSW ONLY) - Local Travel Required

Job Summary

Conduct comprehensive member assessments via in-person home visits and complete waiver enrollment or disenrollment processes. Develop and implement care plans in collaboration with members, caregivers, and healthcare professionals to address specific needs and goals. Perform ongoing monitoring of care plans, evaluate effectiveness, document interventions, and suggest changes accordingly. Facilitate interdisciplinary care team meetings for service approvals or denials while utilizing motivational interviewing to support member change. Assess for medical necessity, authorize appropriate waiver services, and identify barriers to care to provide coordination assistance for psycho-social, financial, and medical obstacles. Requires valid driver's license, reliable transportation, and 25-40% estimated local travel within Mercedes and La Feria, TX. Monday through Friday, 8 AM – 5 PM CST.

Required Qualifications

  • TX licensed LVN, LBSW or LMSW
  • residing in the service delivery area of Mercedes and La Feria, TX
  • 2 years of health care experience
  • at least 1 year of experience working with persons with disabilities/chronic conditions long-term services and supports (LTSS)
  • 1 year of experience in care management
  • experience in a medical and/or behavioral health setting
  • equivalent combination of relevant education and experience
  • Clinical licensure and/or certification
  • active and unrestricted license in state of practice
  • bachelor's degree in a health care related field
  • Valid and unrestricted driver's license
  • reliable transportation
  • adequate auto insurance for job related travel requirements
  • Demonstrated knowledge of community resources
  • Ability to work within a variety of settings
