Job Summary
Conduct telephonic and face-to-face assessments to evaluate member needs and requirements for optimal wellness. Guide families toward appropriate resources and create care plans to address physical, environmental, and psycho-social health issues. Resolve barriers to effective care by collaborating with providers and the transdisciplinary team to ensure continuous progress toward desired outcomes. Submit incident reports and monitor care through ongoing evaluations. Requires 3+ years of experience as an LPC, LCSW, LAC, or psychiatric RN with applicable state licensure. Full-time role with a 40-hour week, Monday through Friday, 8:00 AM to 5:00 PM, with occasional travel for training.
Required Qualifications
- 3+ years experience as LPC, LCSW, LAC or psychiatric RN
- Applicable State licensure in field of study
- Intermediate to Advanced computer skills
- Experience with Microsoft Office
- Must be able to work a 40 hour work week
- Monday through Friday 8:00 AM to 5:00 PM
- At minimum, a download speed of 25 Mbps and an upload speed of 10 Mbps
- Wireless, wired cable or DSL connection
- Dedicated space lacking ongoing interruptions
- To protect member PHI / HIPAA information
- Occasional travel to Humana's offices for training or meetings
Desired Qualifications
- Must be passionate about contributing to an organization focused on continuously improving consumer experiences
- Bilingual a plus
- Previous managed care experience
- Certification in Case Management
- Experience working with children and adolescence
