Job Summary
Support individuals and families affected by substance use through intensive engagement, facilitating individual and group interventions to promote recovery and social reintegration. Collaborate with local organisations, mutual aid groups, and treatment teams to identify needs and manage onward referrals to specialist services. Liaise with third sector and statutory partners to ensure access to appropriate support, while maintaining defensible documentation and adhering to information governance safeguards. Work across multiple locations in Kirklees on a part-time basis including evening shifts, prioritising personalised care plans within a compassionate, values-led team environment.
Required Qualifications
- Enhanced DBS check
- Good understanding of substance use issues
- Experience of working with people
- Excellent communication skills
- Skills in defensible documentation
- Proactive ability to work on own initiative
- Excellent time management and prioritising skills
- Good working knowledge of family services and frameworks
- Knowledge of interventions for whole family practices
- Ability to work in partnership with a wide range of professionals, agencies, and internal/external stakeholders
- Ability to manage change successfully
