Job Summary
Establish and maintain trusting, culturally responsive relationships with assigned children and families, serving as the primary point of contact for ongoing support, advocacy, and resource navigation. Conduct comprehensive family assessments and CalAIM Enhanced Care Management (ECM) evaluations to develop individualized service plans and care goals. Coordinate multidisciplinary services across healthcare, housing, education, and behavioral health providers while facilitating referrals and participating in case conferences. Maintain complete documentation in agency systems and monitor family progress toward care plan objectives. Manage a caseload of approximately 100 families with a focus on trauma-informed, strengths-based approaches to improve health and educational outcomes.
Required Qualifications
- A high school diploma or equivalent (GED)
- Associate's degree in Social Work, Human Services, Child Development, Public Health, Psychology, Family Studies, Public Administration, or a closely related field
- Minimum three (3) years of progressively responsible experience providing family support, care coordination, case management, social services, healthcare navigation, early childhood services, or community-based programs
- Experience working with culturally diverse, low-income, and underserved children and families
- Demonstrated experience coordinating services across multiple agencies, healthcare providers, and community partners
- Experience maintaining confidential case documentation and managing complex caseloads
- Bilingual English/Spanish
- Valid California Driver's License
- Reliable transportation
- Proof of current automobile insurance
- Ability to maintain the highest level of confidentiality when handling participant, family, medical, behavioral health, and organizational information
- Bend: Flexion of the upper trunk forward while standing and knees extended, or knees flexed when sitting
- Lift: Exertion of physical strength to move objects 10-25lbs from one level to another
- Carry: Hold or rest weighted objects 10-25lbs directly on hands, arms, shoulders, or back while moving from one location to another
- Climb: Ascend/descend with gradual or continuous progress, using both hands and feet
- Push: Exertion of force on or against an object (weight/size) to move it from one location to another
- Feel: Perceiving attributes of objects, such as size, shape, temperature, or texture by touching with skin
- Travel: Travel to sites using a personal vehicle may be required based on job duties
Desired Qualifications
- Bachelor's degree in Social Work, Public Health, Human Services, Child Development, Healthcare Administration, Psychology, or a closely related field
- Equivalent combinations of education, lived experience, and progressively responsible experience
- Experience working in Head Start, Early Head Start, CSPP, CalAIM Enhanced Care Management (ECM), Medi-Cal Managed Care, behavioral health, housing services, or other publicly funded human services programs
- Working knowledge of CalAIM Enhanced Care Management (ECM), social drivers of health, and whole-family service delivery models
- Experience utilizing electronic case management systems, including ChildPlus, Brightwheel, EmpowOR, SharePoint, or similar client information systems
