Job Summary
Provide in-home supports to families with children under six through the Growing Families Home Visitation Program, delivering parenting education, guidance and support via collaborative goal setting; plan and prepare individualized visits that respond to family needs, work with families to apply new strategies and skills, link them to community resources and supports, and complete required assessments, service plans, case notes, and discharge documentation.
Required Qualifications
- Degree or Diploma in social services/human services or related field
- Minimum of three (3) years of experience working one-on-one with families
- Solid understanding of child development
- Experience working with a diverse population that may be experiencing mental health, poverty, homelessness and or addiction issues
- Solid knowledge of community resources and supports
