Job Summary
Build trusting, strengths-based relationships with families and develop individualized family partnership agreements. Connect families to community resources, healthcare providers, and social services while supporting them through challenges with a calm, resourceful presence. Plan and participate actively in family events, parent committee meetings, and community outreach activities. Maintain accurate documentation and case records in compliance with program requirements. Work with families to value and understand the benefits of full family engagement and a 2-generational approach within an education setting.
Required Qualifications
- Associate's degree in Social Work or related field
- 1+ year in social services
- Ability to hear the conversational voice, with or without a hearing aid
- Ability to see and read newsprint with or without corrective lenses
- Ability to speak and be understood under normal circumstances
- Must be able to drive including getting into and out of vehicle frequently and sitting for brief periods of time
- Ability to climb stairs
- Ability to bend and lift and reach
- Ability to function within guidelines of Agency's core values and identification with Agency mission
