Family Support Specialist

Job Summary
Conduct family support services by coordinating care plans, conducting home visits, and providing case management for clients. Maintain accurate documentation in electronic systems and communicate regularly with families, caseworkers, and medical providers. Drive to client locations for appointments and transport, including frequent entry and exit from vehicles. Perform physical tasks such as climbing stairs, bending, and lifting materials as needed. Operate within agency core values and mission guidelines while utilizing strong interpersonal and organizational skills. Requires ability to hear and speak clearly, read newsprint, and function effectively in diverse settings.
Required Qualifications
- Associate's degree in Social Work or related field
- 1+ year in social services
- Ability to hear the conversational voice, with or without a hearing aid
- Ability to see and read newsprint with or without corrective lenses
- Ability to speak and be understood under normal circumstances
- Ability to drive including getting into and out of vehicle frequently
- Ability to sit for brief periods of time
- Ability to climb stairs
- Ability to bend and lift and reach
- Ability to function within guidelines of Agency's core values
- Identification with Agency mission
