Job Summary

Conduct home visits virtually or in-person to assess family needs, provide case management services, and promote self-sufficiency while utilizing child development curriculum and floor time to support parent-child relationships. Perform outreach via calls, texts, and mailings; maintain assessments and documentation per agency and funding requirements; and coordinate with community services. Participate in weekly supervision, Healthy Families conferences, and continuous quality improvement activities. Adhere to the agreed schedule, which includes a strict 90-day in-office period, and ensure a functional home working space with adequate internet.

Required Qualifications

Associates or Bachelor's Degree in Social Work or related behavioral health field
High School Diploma and two years of experience in related field
Ability to interact effectively with families
Knowledge in family life skills
Knowledge of child development
Experience in working with at-risk children and their families
Knowledge of and sensitivity to cultural differences
Ability to communicate effectively both orally and in writing
Ability to work effectively on a family support team
Flexibility in scheduling to meet the needs of program participants (to include evening or weekend visits)
Must be eligible and able to obtain/maintain a level 1 fingerprint clearance card
Must be certified in CPR/First Aid
Location: must be able to provide direct service to the designated client base in the Yuma County area
Maintain a functional and safe home working space with adequate internet connection for the job
Must have a valid Arizona driver's license, vehicle and meet agency liability insurance requirements
