Job Summary
Provide case management and crisis intervention for at-risk youth and their families, using Motivational Interviewing and facilitating the Strengthening Families Program (SFP) virtually, in-office, or at other locations as needed. Coordinate referrals, service planning, and documentation; respond to crises with independent judgment; develop and monitor treatment plans; assist with emergency shelter placements or respite care; maintain confidentiality and comply with state/federal standards; travel to client homes and partner sites; participate in supervision, meetings, and staff development; maintain required certifications and driving responsibilities; support linguistic and cultural needs in a fast-paced, multidisciplinary team environment.
Required Qualifications
- First Aid and CPR certificates
- Valid Texas Driver’s license with clear driving record and reliable transportation
- Pass a pre-employment drug screen and random drug screens
- Provide proof of work eligibility status upon request
- Pass a pre-employment and biennial criminal background and motor vehicle check (fingerprints if lived out of state in last 5 years)
- English (United States) proficiency
