Job Summary
Conduct regular home visits using the Healthy Families America evidence-based model to build trusting, strengths-based relationships with expectant parents and families with young children. Provide parenting education, assist families in creating individualized goals, and connect them with community resources, healthcare providers, and schools. Partner with medical providers and community organizations to ensure coordinated support while maintaining timely, accurate, and confidential documentation. Complete assessments, service plans, and participate in reflective supervision and staff meetings.
Required Qualifications
- Associate's degree
- Bachelor's degree in Social Work, Human Services, Psychology, Education, Early Childhood, or a related field
- Valid driver's license
- Reliable transportation
- Acceptable driving record
- Required automobile insurance
- Successful completion of criminal and motor vehicle background checks
Desired Qualifications
- Bachelor's degree in Social Work, Human Services, Psychology, Education, Early Childhood, or a related field
- One to two years of experience in home visiting, parenting education, family support, infant/toddler development, or related services
- Excellent verbal, written, and interpersonal communication skills
- Strong organizational skills with the ability to manage multiple families and priorities
- Ability to maintain accurate documentation and confidentiality
- Proficiency with Microsoft Office, including Outlook, Word, and Excel
- Experience with Office 365 and SharePoint
