Job Summary
The Family Support Specialist II provides comprehensive case management, referrals, and coordination with community and supportive services to promote economic self-sufficiency for families and enhance the quality of life for seniors and people with disabilities. Responsibilities include conducting intake and assessments, enrolling residents in family support programs, developing Development Plans, enabling access to social services, advocating for service delivery for up to 100 individuals, tracking resident data in the LEARN system, attending community and program-related meetings, and delivering resources and knowledge for clients to access capital and financial literacy resources through in-person and virtual seminars. Occasional nights/weekends and travel up to 25% are required. The role supports the development of the USI CDFI client base and requires collaboration within a multidisciplinary team.
Required Qualifications
- Bachelor’s degree in social work, Sociology, Psychology, or related field
- 3+ years of experience as a case manager or in direct practice with low-income households; health services experience a plus
- Valid driver’s license and automobile availability for work use
- Commitment to strengths-based, family-focused service planning and understanding of program philosophies
- Ability to handle confidential information professionally
- Independent and proactive work ethics, with strong team collaboration skills
- Availability for evening and weekend functions as required
- Strong verbal and written communication skills for diverse communities
- Effective real-time priority management
- Excellent organizational, planning, and time management skills
- Ability to excel in fast-paced, culturally diverse environments
- Proficiency in MS Office Suite (Advanced Excel) and other common software
