Family Support Specialist

Conduct home visits to provide emotional support and practical assistance to families facing crises or transitions. Coordinate with community resources to connect clients with essential services such as food, housing, and healthcare. Maintain detailed case files documenting client needs, service utilization, and progress toward goals. Complete required documentation and reporting within established deadlines.
