Job Summary
Conduct comprehensive assessments and engage children and families to develop and implement individualized treatment plans and contracts. Modify plans as needed during treatment, manage discharge planning for community follow-up, and provide ongoing case management services. Collaborate with schools, courts, hospitals, and other community agencies to ensure comprehensive care, while maintaining accurate clinical documentation and reports. Represent The Village in community groups and committees as assigned. Participate in agency task forces and problem-solving teams to support organizational goals.
Required Qualifications
- BA in social work, sociology, psychology or comparable human service program
- Excellent oral and written skills and ability to communicate comfortably with children, parents, and school personnel
- Organized and able to work independently with minimal supervision
- Flexible and able to multi-task
- Cultural competency to serve targeted populations
- Commitment to Results/Performance Based Accountability
- May be required to maintain valid driver's license and ability to drive or otherwise obtain transportation to provide services throughout the service area
- Proficient with Microsoft Office and spreadsheet applications
- Possess solid general computer skills
Desired Qualifications
- 2-3 years' experience in a comparable position in a human services agency preferred but not required
- Bilingual skills preferred
- Experience working with inner-city children
- Familiarity with specific applications used by The Village a plus
