Job Summary
Meet with families to assess needs and support treatment planning for children with severe mental illness. Provide coaching, emotional support, and skill-building while linking caregivers to community resources and services. Share relevant lived experience to promote trust and connection, helping families develop self-advocacy and problem-solving abilities. Maintain accurate documentation in the electronic health record. Required Qualifications
- High school diploma or GED
- Ability to work in a community-based setting
- Ability to pass all required background checks, including Enhanced Fingerprinting with ID DHW
- Valid driver's license
- Reliable transportation
- Proof of auto insurance
