Job Summary

Family Support Navigator within the Coordinated Family Supports program provides case management, assistance with navigating and accessing services, identifying and overcoming barriers, planning, education, and related supports for adults who are clients of the San Diego Regional Center in Imperial County and their families. Responsibilities include maintaining active caseloads, completing assessments, developing written support plans, regular client contact, connecting families with resources, documenting services, participating in supervision and team meetings, community outreach and presentations, assisting with reporting and invoicing, delivering culturally and linguistically appropriate services, and collaborating with staff and providers. The role requires bilingual English/Spanish communication, relevant human services education or equivalent experience, and the ability to travel within the Imperial Valley as needed.

Required Qualifications

  • Bilingual (English/Spanish)
  • Associate-level degree in a human services field or 3 years of experience in the developmental disabilities service delivery system; and
  • Bachelor's degree in a human services field (such as psychology, social work, child development, etc.); OR an Associate's degree in a human field and at least 3 years of experience supporting individuals living with IDD
  • At least one year of experience supporting individuals living with intellectual/developmental disabilities (IDD)
  • Knowledge of Imperial County's services, resources, and culture
  • Exceptional interpersonal skills, positivity, attention to detail, flexibility, and ability to work in a changing environment
  • Ability to communicate in primary language of client and family
  • Excellent planning, organizing, problem-solving, time management, and coordinating skills
  • Ability to work with minimal direction and supervision
  • Demonstrated ability to maintain confidentiality
  • Strong positive viewpoint towards individuals with developmental disabilities, mental health challenges, and history of substance use and their families
  • Natural ability to build and maintain relationships with diverse stakeholders
  • Ability to get along and work effectively with others
  • Excellent written and verbal communication
  • Regular, predictable attendance
  • Medi-Cal Peer Support Specialist Training completion preferred
  • Intermediate to advanced knowledge of Microsoft Office (Word, Excel), email, and web navigation
  • Must be willing and able to drive own car; valid CA driver’s license and insurance; satisfactory driving record
