Job Summary
Build and nurture strong relationships with children and families accessing services, helping them complete forms and attend appointments. Support families in navigating external services such as Centrelink, NDIS, and the Department of Communities while maintaining accurate, confidential client records. Work within a multidisciplinary team including paediatricians, nurses, and GPs to assist with intake and case management, ensuring all work is conducted in a culturally safe, family-centred manner. This role is a maximum term full-time contract expiring 12 months from the start date. Join the Derbarl Yerrigan Health Service team, the largest Aboriginal community-controlled provider in Western Australia, dedicated to improving the health, learning, and social wellbeing of Aboriginal and Torres Strait Islander children.
Required Qualifications
- A person of Aboriginal and/or Torres Strait Islander descent
- Completed post-school qualification in a relevant field (Health, Social Work or equivalent)
- Competency with Microsoft Office and client management systems
- Successful Criminal Record Screening Clearance
- Successful Working with Children Check
- Evidence of a current C or C-A Class driver's licence or other specialised licence class
