Job Summary
Family Stabilization Case Manager responsible for implementing emergency assistance and case management programs, conducting initial client interviews and intake, assessing client needs and developing self-sufficiency plans, coordinating with CAPWN programs and external agencies, providing outreach and training sessions, and ensuring compliance with HIPAA and funding guidelines; position requires office-based work with occasional travel, strong communication, trauma-informed approach, and ability to document and report on client progress.
Required Qualifications
- AA in Human Service, Social Work, Psychology or related field OR 3 years’ experience in a Human Service agency working with clients facing insufficiencies
- Bachelor’s Degree in Human Services, Social Work, Psychology or related field OR 6 years’ experience in a Human Service agency working with clients facing insufficiencies
- Minimum qualifications include Grade: 2, 1 year experience in a human service program or agency working with clients facing insufficiencies
- Proficiency in verbal communication and written documentation
- Valid driver’s license and insured vehicle preferred
