Family Services Coordinator

Job Summary
Provide in-home supportive services, transportation, parenting education, counseling, community resource referrals, and advocacy to youth and families. Coordinate and monitor mental health, medical, educational, and social services while establishing strong relationships with foster parents and families of origin. Deliver safety assessments, crisis response, and behavioral stabilization as required. Travel daily to community-based settings, including schools when necessary, and work two evenings weekly until 7:00 PM. Maintain a valid driver's license and reliable transportation for all duties.
Required Qualifications
- Degree in Social Work or comparable Human Services field from an accredited institution
- Valid driver's license
- Reliable transportation
- Good driving record
- Minimum automobile insurance coverage of $100,000/$300,000 bodily injury liability
- Willingness to work flexible and non-traditional hours with afternoon and evening availability
- Proficient use of desktop and laptop computers, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs
- Must be available to work two evenings each week until 7:00PM when needed
- Travels daily, to provide community-based services to, and on behalf of, youth and families
Desired Qualifications
- 2 years of work experience working in direct service with youth and families
