Family Services Advocate Worker -Mission, Texas ($22.33/HR)

Implement Health Transitional and Family Services at the Center including eligibility, recruitment, selection, enrollment, attendance (ERSEA), home visits, newborn home visits, family needs assessments, referrals, and case file reviews. Participate in case review processes addressing health, nutrition, and disabilities to determine children's status and coordinate MSHS services with community agencies. Assist with family services management using a case management systems approach to identify family strengths and needs through advocacy and training. Conduct parent orientations, obtain consents for EPSDT services, and coordinate transportation for health and dental appointments. Develop Family Partnership Agreements outlining goals and timelines, and utilize the PFCE Framework to offer family houses, parent trainings, and volunteer programs. Ensure timely data entry of services, submit monthly reports, and maintain the parent board with meeting schedules and community event information.
