Family Services Advocate Worker -Alto Bonito, Texas ($22.33/HR)

Implement Health Transitional and Family Services at the Center, including eligibility, recruitment, enrollment, attendance, home visits, newborn home visits, family needs assessments, referrals, and case file reviews. Participate in case review processes to determine children's health, special needs, and nutrition status by performing file reviews and obtaining current health or dental information. Assist with family services management using a case management systems approach to identify family strengths and immediate, short-, and long-term needs through advocacy, training, and coordination of services. Implement and monitor ongoing recruitment plan activities to ensure funded enrollment is met, including establishing 10% enrollment of children with disabilities. Address health, nutrition, and disabilities issues by consulting with parents, sending "symptoms" letters, conducting home visits when concerns are suspected, and sending referrals as needed. Conduct parent orientation, coordinate document collection such as immunizations and health histories, and develop Family Partnership Agreements outlining family goals, school readiness objectives, and timelines. Utilize the Parent, Family, Community, Engagement (PFCE) Framework to offer family houses, parent trainings, volunteer programs, and parent surveys while encouraging family advocacy and participation. Assist families in accessing community resources through effective referrals, monitor cases for follow-up, and obtain consents for EPSDT, health, or dental services. Coordinate with the Child Development Coordinator on accident reporting, medication administration, hygiene techniques, and individualization for children with special needs. Maintain an effective filing system safeguarding confidential information and ensure timely data entry of services into the family Data Base System. Report on suspected child abuse and neglect in accordance with state law and UMOS policies. Attend center and regional staff meetings, coordinate meetings and trainings, and perform other duties as assigned.
