Family Service Coordinator-Supportive Housing

Job Summary
Develop and write client plans addressing emotional, physical, social, and cultural issues to reduce housing barriers while assisting clients experiencing homelessness with housing search and placement activities. Work directly with an integrated team from CAP, the County, and other organizations to determine referred program eligibility and establish relationships with landlords and short-term residences to link clients to local resources. Maintain composure under stressful conditions and participate in local committees such as the Housing Support Meeting and Heading Home Committee. Requires a four-year social work degree, one year of experience, and knowledge of Trauma-Informed Care principles. Monday-Friday 8am-4:30pm.
Required Qualifications
- A Four-year degree in social work or related field
- one year of associated experience
- equivalent combination of education and/or experience
- Experience working with individuals or families who are unhoused
- Experience working with individuals or families from various social backgrounds
- knowledge of the principles of Trauma Informed Care
- knowledge of Person Centered Theory
- knowledge of Housing First
- experience working with mental health populations
- experience working with chemical dependency populations
- the principles of record keeping
- the principles of record management
- the ability to quickly adhere to housing policies, procedures and applicable CAP software
