Family Service Coordinator

Job Summary
Coordinate and monitor the delivery of early intervention services for families of children from birth through age three, acting as the single point of contact for service access and IFSP development. Facilitate eligibility determinations, scheduling, and multidisciplinary meetings while ensuring timely transitions and funding coordination. Assist parents in gaining access to required supports and explain procedural safeguards, maintaining all documentation and equipment in proper working order. Adapt to varying service delivery settings including clinical, community, and home environments, and respond to emergency situations with competence. Travel within the southwest region of New Mexico as needed to support families and participate in departmental staff development activities.
Required Qualifications
- A bachelor's degree in social work, counseling, psychology, nursing, special education, early childhood education, or any other closely related field
- A bachelor's degree in another field plus two (2) years of direct experience in serving individuals with disabilities and/or families
- Training for family service coordination is in accordance with NM FIT Program standards within one (1) year of hire
- Completion of the web-based family service coordination training every three (3) years
- Receive reflective supervision at least once (1) every month
- Must be able to adapt to a variety of settings and be willing to work in clinical, community, and home settings
- Must be able to travel to and from areas within the southwest region of New Mexico
- Must be able to work in crowded work areas
- Must be able to work on one's feet for extended periods
- Must be willing to be exposed to hazardous conditions, contagious diseases, and chemicals to repair/adjust patient equipment
