Job Summary
Provide in-home prenatal and parenting education and support for parents and children birth to 5 years old through individualized, home-based visits. Use the curriculum to educate on prenatal health, breastfeeding, parent-child interactions, child development, safety, sexual health/family planning, and parent/child health. Plan personalized lessons, complete documentation and assessments, coordinate service plans and discharge summaries, travel to appointments, conduct remote visits via Zoom/Facetime/phone, participate in community meetings, refer families to local resources, respond to emergencies, advocate with other agencies, and participate in CQI and supervision. Flexible schedule with some evenings or weekends; supervision weekly; travel to diverse settings as assigned; require defensive driving and license checks as part of agency requirements.
Required Qualifications
- High School Diploma or equivalent
- Associate's degree or 3-5 years related work experience
- Bachelor’s degree in Social Work or related field preferred
- Strong direct service professional skills
- Ability to develop respectful relationships with clients
- Ability to work collaboratively with coworkers and other professionals
- Ability to work autonomously and in crisis situations
- Transportation and insurance that meets agency requirements
- Defensive driving course completion
- Nebraska driver’s license with ≤3 points or comparable standard
