Job Summary
Conduct intake and screening for families to assess eligibility and identify service needs. Develop individualized plans with families to promote self-sufficiency and long-term stability while providing navigation, information, and referrals to appropriate community resources. Coordinate with internal programs and external service providers to connect families with relevant support, actively networking to stay current on local offerings. Design and execute family outreach plans to engage children and families in service programs. Maintain records of referrals and navigation outcomes to track progress and facilitate program exchange. Assist with grant and monthly reporting, ensuring accurate data entry and file management. Contribute to shared learning initiatives with state-wide family navigators and attend community meetings to foster information sharing.
Required Qualifications
- 1 year experience in community and/or family service
- One-step Mantoux TB skin test prior to start of employment, unless has written proof of a negative test within past 12 months
- A valid driver's license and a favorable driving record, with no more than three violations within the past three years
- Access to a reliable, insured vehicle for work-related travel
Desired Qualifications
- Lived experience navigating family support systems/services
- Associates degree in family or social services, or a related field
- Bilingual
