Job Summary
Conduct comprehensive nursing assessments, manage patient care plans, and provide education for conditions including diabetes, prenatal care, and chronic disease. Perform clinical tasks such as injections, dressing changes, and Pap tests while assisting physicians and facilitating patient navigation through the health system. Develop self-management programs, implement health screenings, and ensure accurate record maintenance to improve quality of care. Work collaboratively within a family practice team to prioritize needs and support seamless patient flow.
Required Qualifications
- LPN designation with current registration with CLHA (previously CLPNA) in Alberta
- Current standard BLS Provider certificate
- Three years related nursing experience
- A current and clear Criminal Record Check, including a Vulnerable Sector Check
- A valid driver's license and access to a vehicle for travel
Desired Qualifications
- primary care environment
- Clinical experience in primary care nursing, community, gerontology and/or chronic disease management
- Knowledge in health promotion, health screening, assessment and education
- Comprehensive knowledge of normal laboratory values and diagnostic imaging
- Experience in caring for patients with chronic, complex, and/or specialized needs
- Experience with Electronic Medical Records systems and good computer skills
