Job Summary
Provide comprehensive primary care to underserved communities in the metro Phoenix area through a Patient Centered Medical Home model. Deliver Family Medicine, Internal Medicine, Pediatrics, Dental, and Behavioral Health services across nine clinics in Maricopa County. Maintain valid Arizona medical licensure, DEA certification, and Level One fingerprint clearance while adhering to core values of inclusion, nurture, service, purposeful integrity, resilient engagement, and engaged community support.
Required Qualifications
- Graduate of an accredited medical school program
- Completion of an accredited United States Family Practice Residency program
- Board certified or board eligible
- Valid Arizona medical license
- Valid DEA certificate
- Demonstrated understanding of clinical methods and techniques
- Ability to meet and deal effectively with clients, associates and the general public
- Basic business English, spelling and punctuation skills
- Basic computer efficiency
- Valid AZ driver's license
- Commitment to Adelante Healthcare mission, vision, and values
- Competency in dealing with people of various cultures and social status
- Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)
- Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
