Job Summary
Coordinate and implement family support services through the CFTSS and CHCBS plan process using a strength-based, family-centered approach. Provide individual or group CPST and PSR services in the home or community to support recovery goals for youth with behavioral and emotional challenges. Deliver evidence-based therapeutic and crisis intervention, including supportive counseling, psychoeducation, and problem behavior analysis to prevent out-of-home placements and hospitalizations. Develop and document integrated treatment plans, progress notes, and assessment tools while maintaining productivity targets and monthly contact requirements. Educate clients and families on CFTSS and HCBS Waiver services to support informed decision-making. Complete all required trainings and attend department meetings as assigned.
Required Qualifications
- Minimum of High school diploma or equivalent
- Completion of additional training requirements including 12 hours of Introductory Training in the Addictions required within 30 days from date of hire
- Valid NYS driver's license
- Reliable transportation
- CPR certification within 6 months of employment
- Must be able to physically move/react quickly, including climbing stairs, in response to situations on all interior floors of the residence and outside
- Valid NYS drivers license with driving record acceptable to BHSN insurance carrier
Desired Qualifications
- Experience in or knowledge of alcohol/substance abuse delivery system
- Strong interpersonal, communication, time management and organizational skills
- Proficient in Microsoft Office software and in maintenance of electronic records/charts
- Excellent interpersonal and communication skills aimed at providing excellent customer service to program clients as well as other agency staff
