Job Summary
Establish and maintain strong relationships with families serving as a consistent point of contact and source of support throughout the Wraparound treatment process. Empower families to actively participate in care planning and decision-making while collaborating with clinicians, care coordinators, and therapists to facilitate seamless service delivery. Provide education on available services and treatment options, acting as an advocate to help families navigate complex systems. Utilize a strengths-based approach to teach caregivers effective strategies for managing challenging behaviors and enhance their coping skills. Maintain accurate documentation of all interactions, progress, and interventions according to program guidelines. Full-time, 40-hour schedule with flexible hours between 8 a.m. and 8 p.m. to meet family needs.
Required Qualifications
- Bachelor's degree in human services field
- Associate's degree in human service field
- High school diploma/GED
- One year of experience working with the target population
- One year experience working with children/adolescents/transition age youth
- Minimum of two years experience working with children/adolescents/transition age youth
- Experience or experienced being caregiver living with and caring for a child with significant needs
- Experience or experienced being caregiver living with and caring for a child or youth with mental health needs and/or neurodevelopmental needs
- Experience in navigating any of the child and family-serving systems
- Experience teaching family members who are involved with the child and family-serving systems
