Job Summary
Family Nurse Practitioner (FNP) licensed in Oklahoma, delivering virtual wellness visits to patients in their homes and coordinating with in-home Sprinter team members to perform health assessments, medication reviews, cognitive screenings, and preventive care interventions. Responsibilities include reviewing caseloads, navigating schedules with clinical in-home staff, conducting comprehensive patient evaluations, and providing patient-centered education and care coordination. Must be board-certified as an FNP, hold an active Oklahoma license, be able to work autonomously, and demonstrate professionalism, HIPAA-compliant communication, and strong written/verbal skills.
