Job Summary
Conduct comprehensive in-home health risk assessments to identify active and chronic disease conditions, determine physical, mental, and social needs, and take patient vital signs. Perform diagnoses, provide written documentation per NCQA standards, and place case management referrals while communicating with patients, caregivers, agency nurses, and providers. Maintain patient confidentiality, observe OSHA Universal Precautions, and attend required meetings and professional development activities. Requires an active, unrestricted nurse practitioner license, board certification, current Medicare/Medicaid enrollment, valid driver's license, and ability to drive up to 150 miles daily. Adapts to varying weather conditions and residential environments with full range of body motion for lifting up to 50 pounds.
Required Qualifications
- Active/unrestricted nurse practitioner license to practice in coverage states
- Board certification in one of the following: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP) or National Commission on Certification of Physician Assistants (NCCPA)
- Active BLS Certification
- Current enrollment in Medicare/Medicaid
- Must maintain a valid driver's license and good driving record
- Outstanding EHR skills

- Geriatric training/experience
- Skill in teamwork and maintaining effective working relationships with patients, medical staff, and the public
