Job Summary
Deliver full-scope outpatient primary care to a loyal, relationship-driven Wayne County patient panel, managing acute and chronic conditions across all life stages from pediatrics through geriatrics. Partner with a collaborative, community-focused care team to coordinate patient needs while utilizing shared clinical systems to support seamless, high-quality care delivery. Serve as a consistent, accessible provider for families who depend on local healthcare in a community setting with no hospital rounding. This role offers Board-Certified Family Nurse Practitioners a chance to build a meaningful practice backed by a larger academic-affiliated health system, situated between Rochester and Syracuse with access to the Finger Lakes region's outdoor recreation and small-town communities.
Required Qualifications
- Board-Certified Family Nurse Practitioner (FNP-BC)
- Master's degree in Nursing (MSN) with NP certification
