Job Summary
The Family Nurse Practitioner (NP) will provide quality primary health care to patients in a team-based setting, collaborating with staff physicians to assess/evaluate, diagnose, treat, and educate patients across pediatric to adult populations. Responsibilities include managing acute and chronic conditions, prescribing medications, referring to specialists, coordinating patient care with nursing and support staff, performing in-office procedures, and contributing to outreach and quality-improvement initiatives. The role requires strong communication, trauma-informed de-escalation techniques, proficiency with electronic health records, and bilingual ability (English/Spanish or other threshold language) preferred. Location: East Fairfield, CA and Fairfield, CA; full-time position at 36 hours/week (potential to 40 hours/week) with benefits.
Required Qualifications
- Graduate of an accredited Master of Science in Nursing
- Current California Family Nurse Practitioner certification
- Must have or obtain current California medication furnishing license
- Must have or obtain current DEA License
- CPR Certification required
- Experience in a community healthcare center preferred
- Bilingual (English/Spanish or other threshold language) preferred
