Job Summary
Provide day-to-day leadership and direct patient care to Nurse Practitioners and Physician Assistitors across New Jersey. Conduct comprehensive palliative care assessments, manage complex symptom needs, and facilitate advance care planning with patients and families. Mentor team members, support onboarding, and lead interdisciplinary rounds while monitoring clinical quality metrics and documentation compliance. Collaborate with physicians, social workers, and care coordinators to improve patient outcomes and reduce avoidable hospitalizations. This role combines clinical practice with operational oversight in a rapidly growing organization dedicated to serious illness management.
Required Qualifications
- Current New Jersey Nurse Practitioner or Physician Assistant license
- National board certification as an NP or PA
- Active DEA license and CDS registration
- Minimum of 5 years of clinical experience in palliative care, hospice, geriatrics, oncology, family medicine, internal medicine, or complex chronic disease management
- Strong leadership, communication, and interpersonal skills
- Ability to work independently and effectively within interdisciplinary care teams
Desired Qualifications
- Prior leadership, mentoring, or supervisory experience
- Certification in Hospice and Palliative Care (ACHPN or equivalent)
- Experience with home-based care, value-based care, or population health programs
- Familiarity with telehealth platforms and EMR systems
- Bilingual language skills
