Job Summary
Manage a patient panel with a broad array of patient needs, including acute, chronic, and well visits, both in-office and at testing centers, with occasional tele-health visits. Participate in weekly clinical rounds and collaborate with in-office and virtual clinical teams. Utilize clinical training to perform in-office procedures and pursue additional state licensure/credentialing for virtual primary care in additional states. Requires completion of an accredited FNP or PA program with national certification and CT state licensure by start date; minimum 2 years outpatient primary care (or 1 year plus fellowship/urgent care) within the past 5 years. Base in CT offices (Darien, Westport, or New Canaan) with a comprehensive benefits package and competitive compensation.
Required Qualifications
- Completed an accredited FNP or PA program with a national certification
- State licensed in Connecticut by start date
- In the past 5 years, practiced as an Advanced Practitioner for at least: 2 years in outpatient primary care seeing patients of all ages (0+), OR 1 year in outpatient primary care seeing patients of all ages (0+), plus either a 1 year primary care fellowship or 1+ year in urgent care setting seeing patients of all ages (0+)
