Family Nurse Practitioner or Physician Assistant - House Calls (Full-Time)

Deliver comprehensive primary care services directly in patients' homes throughout Middleton and surrounding North Shore communities, including older adults, those with chronic illnesses, and individuals facing mobility or transportation challenges. Perform in-home evaluations, annual wellness visits, and chronic disease management while assessing, diagnosing, and treating acute and chronic conditions. Collaborate with physicians, nurses, and specialists to develop treatment plans, order diagnostic tests, and prescribe medications. Coordinate care with home health agencies, rehabilitation providers, and hospice organizations, while educating patients on disease management and healthy lifestyle practices. Maintain accurate electronic medical records and address social determinants of health. Travel throughout the service area using a provided MFM Health vehicle.
