Job Summary

Family Nurse Practitioner sought to deliver primary care and geriatric services in the Onalaska/La Crosse/La Crescent area with hybrid in-person work. Responsibilities include conducting comprehensive patient assessments, diagnosing and managing acute and chronic conditions (diabetes, hypertension, COPD, asthma), completing admission histories and physicals, performing medication reconciliation, developing individualized treatment plans focused on prevention and chronic disease management, educating patients and families, and collaborating with physicians, nursing staff, therapists, and interdisciplinary teams. The role supports behavioral health needs, maintains timely clinical documentation, and requires travel between partner facilities within the service area. Requires active FNP credential, Wisconsin license (or ability to obtain), and DEA registration (or ability to obtain). Travel between facilities and hybrid work arrangement are noted.

Required Qualifications

  • Master's or Doctorate degree in Nursing from an accredited program.
  • Current Family Nurse Practitioner certification.
  • Active Wisconsin Nurse Practitioner license (or ability to obtain prior to start date).
  • DEA registration (or ability to obtain prior to start date).
  • Excellent clinical judgment, communication, and interpersonal skills.
  • Experience in primary care, geriatrics, skilled nursing, or long-term care is preferred but not required.
  • New graduates with strong clinical training and a passion for geriatric care are encouraged to apply.
