Job Summary
Provide evidence-based Primary Care to patients at the South Bronx Health Center as part of a collaborative team including nursing, health education, nutrition, social work, and behavioral health services. Deliver medical care to the underserved communities of Brooklyn, Bronx, Manhattan, and Queens, reporting directly to the Medical Director. This Full-time role requires a valid Family Nurse Practitioner N.Y.S. License and at least two years of post-graduate experience in Primary or Ambulatory Care.
