Family Nurse Practitioner - Geisinger Behavioral Health Central

Family Nurse Practitioner (FNP) at Geisinger Behavioral Health Center in Danville, PA. The FNP will perform comprehensive medical assessments, provide medication management and daily patient rounding, and develop and implement treatment plans in collaboration with psychiatrists, psychologists, nurses, therapists, and case managers. Document care in the Epic electronic health record, participate in treatment team meetings, and coordinate care transitions. Communicate with families, caregivers, and community providers to support continuity of care. Population served is 75% adult and 25% pediatric (age 14+). Work environment includes onsite pharmacy and phlebotomy services with Epic as the EHR; rotating 3 days on, 4 days off. Qualifications include a Master's Degree for Nurse Practitioners, PA licensure eligibility, NP certification, and BLS.
