Job Summary
Provide comprehensive primary care to patients across the lifespan, including routine exams, follow-ups, chronic disease management, and sick visits. Order and interpret diagnostic tests, develop treatment plans, and ensure accurate EMR documentation. Perform basic outpatient procedures and manage acute and chronic conditions while engaging in patient education and preventive health initiatives. Collaborate closely with physicians, nurses, and care coordinators for interdisciplinary coordination and participation in team huddles. Uphold organization standards for patient safety, compliance, and evidence-based care while contributing to quality improvement and workflow efficiency. Required Qualifications
- Master's Degree in Nursing
- Completion of an accredited Family Nurse Practitioner program
- Active/Eligible for New York State FNP License
- Board Certification/Eligible as a Family Nurse Practitioner
- Current BLS certification
Desired Qualifications
- Experience in primary care or outpatient practice preferred
