Family Nurse Practitioner (FNP) | Open to new graduates

Conduct primary care for a diverse community across multiple outpatient clinic locations, including same-day in-person and telehealth visits. Convert patient portal messages into appointments, perform Medicare Advantage Annual Wellness Visits, and support coverage when primary providers are unavailable. Lead "team visits" for new patients and serve as "Provider of the Day" for urgent or walk-in concerns while managing inbox tasks. Work five 8-hour days per week from 7:30 AM to 5:30 PM within a collaborative Family Medicine team.
