Job Summary
Provide evidence-based Primary Care to patients at the South Bronx Health Center as part of a collaborative team including nursing, health education, nutrition, social work, and behavioral health services. Deliver medical care to underserved communities across Brooklyn, Bronx, Manhattan, and Queens while reporting to the Medical Director. Maintain a valid Family Nurse Practitioner N.Y. License and at least two years of post-graduate or residency experience in Primary Care or Ambulatory Care settings. This role supports the network's mission to serve 85,000 people annually through its Federally Qualified Health Center programs.
