Job Summary
Provide quality integrated primary health care services and medication management to children, adolescents, and adults in an outpatient setting. Perform as an independent and interdependent member of the medical services team delivering general medical and behavioral health care. Work extensively through the FQHC with adults diagnosed with substance use disorders and/or serious mental illness within residential programs. Maintain prescriptive authority, DEA certificate, and State of Indiana Controlled Substance Registration while holding ANCC Family Nurse Practitioner certification. Complete drug screens, TB tests, and extensive background checks including criminal history and sex offender registry searches. Join a nonprofit provider serving five Central Indiana counties with a hiring bonus up to $5,000.
Required Qualifications
- Master's degree in Nursing or a Doctorate degree in Nursing from a college/university accredited by the US Department Education
- Must be recognized as a Registered Nurse (RN) with Prescriptive Authority by the State of Indiana
- Must have current Drug Enforcement Administration (DEA) certificate and State of Indiana Controlled Substance Registration (CSR)
- Must have ANCC or other certification as a Family Nurse Practitioner
- Must have current BLS certification
- Drug screen, TB test and extensive background checks (including Criminal History, Sex Offender Registry Search, State Central Registry Check, Education Verification, and Professional References)
Desired Qualifications
- One (1) year supervised experience in primary care with Severely Mentally Ill (SMI) and/or substance use disorder population
