Job Summary
Recruit and engage families into the Ascend pipeline by identifying community resources to meet their needs. Refer families to support programs and empower them through education to take the lead in their own Plan of Care. Model nurturing relationships and provide feedback to cultivate connections for accessing services in schools, courts, hospitals, and other agencies. Complete accurate documentation and reports per agency and funding policies. Partner with the supervisor to identify professional growth areas and represent the team in community groups as assigned.
Required Qualifications
- High school diploma
- experience in family skills and community services
- current valid state issued Driver License
- own a vehicle for home visits, community events, etc.
- Strong communication skills both verbal and written
- Solid general computer skills such as typing and navigating websites

- Bilingual - Spanish
